• Broker CRD number (Central Registration Depository) and last four digits of SSN

• Demographic information such as ZIP code and/or the locations at which you plan to hold workshops

• Any other information provided directly by a subscribing firm and/or their firm associates.

What we do with the information we gather.

• Internal record keeping;

• Subscriber communications periodically sent regarding informational updates such as new enhancements, regulatory updates, and training opportunities; and

Medicare Aware will not allow any other non-subscriber to access such personal profiles.

You understand that we may be also required to disclose your personal information but only if required to do so in order to comply with any valid legal process or governmental request (to include, not by way of limitation, in response to a search warrant, subpoena, statute, or court order), or as otherwise provided in the terms of use contained in the applicable Medicare Aware Subscription Agreement and this Privacy Policy.

Medicare Aware may change this policy from time to time by updating this page and will communicate any changes to current subscribers via email to their current email address on file with Medicare Aware.
